Can't activate ps4 as primary

I have only owned one ps4 in my life. On my ps4 console says it's not my primary ps4 and I can't activate it because I have another ps4 activated. On my psn web account management shows I do have a ps4 activated. Why is this happening?

Now the issue is I can't deactivate the current ps4 because the web says I used deactivation feature recently. When? I never owned another ps4.
