Account been hack and i can't activate my account as my primary ps4

Ok so i was playing gta online alone when my system told me that my account has sign in to another PS4 and sign me out!

i don't know how this happen. luckily everything is still here but then i noticed my account is activated on the unknow second ps4

i change my pass. so it won't happen again, i only own 1 ps4 and nobody eles is using it other then me. i try to deactivated all via web but it said "You have deactivated your devices recently and cannot use this feature at this time" Yet i didn't even deactivated anything yet.. Please help!
